Medical Requirements
Medical Documents Required for School Registration
Please bring a Record of a Physical Exam, completed within the last twelve months, to your student’s registration appointment. If your student’s registration appointment precedes an upcoming scheduled Physical Exam appointment, please bring the date of that appointment.
The Record of Physical Exam must include documentation of the following:

A current Immunization Record showing compliance with immunizations required by the State of MA for school entry. (This is typically included on the physical exam form but may also be submitted separately.) Search for School Immunizations on mass.gov.

Documentation of a TB risk assessment* (This is typically included on the physical exam form, but may be submitted separately.)

Additional items required for Kindergarten and Preschool Registrations (only)*

Documentation of lead screening - must include the date of test and results

Documentation of a vision screening with stereopsis

* These items would not prevent a student from starting school, but they are required documents needed within a reasonable time from starting school.
Students who qualify as homeless will not be prevented from starting school if any documentation is missing. Nursing will work with those families as needed to access health services.
