Annual Health Information Updates
The PowerSchool Parent/Student Portal has a Health Information screen. It contains important health information about your student, such as health care contact information, important health problems for the schools to know about, allergies, and medications.
The Nursing Department asks you to please review, for each student, the information on that screen, at the start of each school year, and whenever that information changes. If there are no changes, please let us know that, too, by just clicking Submit at the bottom of the form.
Alternatively, If you would instead prefer to have this information entirely under the control of the school nurse, rather than using the online system, please print a paper form, and submit it to the school nurse.
